Drops from Ranishu Nibbler or similar nearby mobs in Vol'dun Court of Zak'rajan .
After you loot this item you need to go to 46.94 46.57 and find Mysterious Trashpile there.
Use Mysterious Trashpile and you will be ported to Jani world, speak with jani, turn in the item and receive the reward treasure chest.
